Tag it the way the record will read
Commissioning tags are decisions about whether work continues, not comments on how tidy a job looks. Accepted, accepted with comment, incomplete, rejected and safety hold each mean something specific to the next person who reads the record. This drill puts 12 findings in front of you and explains why each tag is the honest one.
What is the difference between incomplete and rejected?
Incomplete means the work is unfinished; rejected means it was finished and it failed. The distinction sounds pedantic until you notice that the two tags say completely different things about a contractor's performance, and both end up in a report somebody reads later. Tagging unfinished work as rejected records a failure that did not happen. Tagging failed work as incomplete hides one that did. The same care applies at the top of the scale: accepting something with a genuine outstanding item, rather than tagging it accepted with comment, quietly deletes the item from the record. Tags are cheap to apply and expensive to get wrong, because the tag outlives the walk and the conversation that produced it.
A site walk turns up these findings. Tag each one. The colour is a decision about whether work continues, not a comment on how tidy the job is.
Scope. Every scenario here is fictional and generalized, set on the fictional Example Site EX1. This is career and interview preparation at awareness level, not a live operating procedure, engineering advice or any form of authorization. On a real site, follow your employer's procedures, your authorized training and local requirements, and if the two ever disagree, the site wins.

Nothing blocks the next level and no comment is needed. Accepted is the only honest tag; downgrading it to be cautious devalues the tag for everyone.

The defect is real and must be logged, but it does not affect performance and does not stop the next level. That is precisely what accepted with comment is for.

This is not a failure, it is unfinished work. Rejecting it would misrepresent the contractor's performance; the honest tag says come back when it is done.

04 During the load bank test the unit trips at 75 percent and will not hold.

It fails its acceptance criteria, so it is rejected and reworked. The distinction from incomplete matters: this was finished and it did not pass.

05 An exposed live conductor is found inside the panel during the walk.

Stop everything. A safety hold outranks the commissioning result entirely, and only the duty holder releases it. Tagging this as rejected would treat a danger as a quality issue.

06 The witness could not attend. The test ran, passed, and is supported by photographs and a signed sheet.

The result stands but the witness requirement was not met, which is a comment the record has to carry. Whether that is acceptable is the owner's call, and the tag is what surfaces the decision.

07 The installed sequence of operations does not match the approved design.

It may well work, but it is not what was approved, so it cannot be accepted against that design. Either the installation changes or the design is formally revised.

Nothing has failed; a check has not been done. That is incomplete, not rejected. Carrying it forward with that row blank is how a functional test ends up testing an assumption rather than the plant.

09 A temporary supply has been run into the panel so the contractor can work. Nobody can say who authorised it or when it comes out.

E · Safety hold
An unidentified energy source with no owner and no removal date is a hazard, not a quality defect. Raise the hold and let the duty holder decide what happens next. Anyone may raise one; only they may lift it, and that asymmetry is the whole point.

10 The test sheet is signed and passed. The photographs attached to it show a different asset ID.

The evidence does not support the result, so the record fails on its own criteria even if the plant is fine. Retest or re-evidence it. Accepting with a comment would leave a signed pass propped up by the wrong photograph, and the next person has no way to see that.

11 Everything on the checklist passes. The contractor asks you to sign now and says the two missing labels will be on by Friday.

The labels are a real outstanding item, so the honest tag logs them rather than deleting them. Signing accepted because the promise sounds reasonable is exactly how items leave a record, and Friday is not a record.

There is nothing to log, so log nothing. Adding a comment to look thorough puts noise into a record whose entire value is that a comment means something.
Now say it out loud
Clicking the right option is the easy half.
In the room you will not be given the options. Cover them, and answer these three out loud, in full sentences, as if somebody had just asked. If a sentence stalls, that is the part to go back and read again.
- 01 Explain the difference between incomplete and rejected to somebody who has never seen a tag, and say what each one tells a reader about the contractor.
- 02 Describe a finding you would raise a safety hold on, and say who is allowed to lift it and why that is not you.
- 03 Say what you would do if a supervisor asked you to tag something accepted while an item was still open, without accusing anybody of anything.
When does a safety hold outrank everything else?
Always, and that is the point of having a separate tag for it. A safety hold is not a severe version of rejected: rejected is a quality judgement about whether the work meets its criteria, while a hold says the situation is dangerous and nobody continues until the duty holder releases it. Filing a live exposed conductor as a quality defect treats a danger as paperwork, and the person who reads that record next has no way to know they should stop. The other reason to keep it separate is authority. Anyone can raise a hold; only a named duty holder can lift one, and that asymmetry is what makes it work on a site where commercial pressure pushes in the other direction.
